General comments
bij ventriculaire/lumbale drain
vancomycine dosering: oplaaddosis 15 mg/kg, vervolgens 35 mg/kg continu per 24 uur, OF oplaaddosis 25 mg/kg, vervolgens 17.5 mg/kg 2 dd. Onderhoudsdosering vancomycine verder afhankelijk van nierfunctie en van gemeten spiegels, zie bij vancomycine (ook voor dosering bij nierinsufficiëntie) en bij TDM vancomycine.
